A Clinical and Statistical Study of the Elderly Primiparas

Abstract
Numerous reports had appeared in the literature citing the danger of pregnancy in elderly primipara. The elderly primipara is generally defined as a women at the age 35 years or greater if she is experiencing the birth of her first viable baby.
192 cases of elderly primiparas at the department of Obsterics & Gynecology, Taegu Fatima Hospital from January 1991 to December 1995 were compared with 200 systemic random sampling chose young primiparas during the same period.
@ES The results were obtained as follows ;
@EN 1. There were 192 cases of elderly primipara among total 35,034 deliveries and the incidence of the elderly primipara was 0.6%.
2. The rate of elderly primipara was increased from 0.3% in 1991 to 0.8% in 1995.
3. Maternal age was distributed from 35 to 44 years. The majority(49.5%) were in age group 35 and 36 years.
4. 171 cases(89.1%) of total cases were vertex presentation and breech presentation was in 20 cases(10.9%) and transverse lie was in 1 case(0.5%).
5. Abdominal delivery was 64.6% in elderly primipara comparing with 20.5% in younger primipara(p<0.01).
6. Concerning the antenatal complications, the incidence of myoma was significantly increased in elderly primipara(p<0.05). The incidences of hypertensive disorder and preterm premature rupture of membranes(PPROM) were higher in elderly
primipara,
but
there were-no statistical significance(p>0.05).
7. Among the indication of Cesarean seetion, highest incidence was elderly primipara(53.2%). Other incidences were abnormal presentation, CPD, fetal distress, multiple gestation and severe preeclampsia, in order of frequency.
8. The incidence of low birth weight(<2,500 mg) was 13.3% comparing with 9.3% in younger primipara(p>0.05).
9. Preterm delivery(before 37 wks) was 9.4% comparing with 4.0 % in younger primipara(p<0.05).
10. The perinatal mortality rate was 26 in elderly primipara comparing with 10 in younger primipara(p>0.05).
11. One cases of 192 cases was congenital anomaly but there was no chromosomal anomaly.
